​Welcome to the new school year! Here are some friendly reminders from your School Nurse to help us provide care and a safer environment to your student while at school.

1) Immunizations:
- In anticipation of all the students returning to campus in the coming months, all students must be up-to-date on the required immunizations for school. Religious and medical exemptions are still acceptable for those students that meet the criteria. The state of Nevada is not waiving the immunization requirements at this time.

2) Medications and medical procedures for when the school building reopens for students:
- A prescription label or doctor’s note, and a completed (parent permission) form CCF-643 (which can be obtained from the Health Office) are required for medication administration on campus.
- Please make sure the medication label or prescription indicates exactly how you want us to provide the medication. We cannot accept “once a day” or “twice a day” orders. Administration times must be specified on the medication label or prescription note (ex: “with lunch” or “ 12:00 pm”). If we are to give an inhaler as needed, ask the doctor to write indications for administration (e.g., “shortness of breath” or “wheezing”) and time interval between administrations (e.g. “every 4 hours” or every “4-6 hours”.) Please ask your doctor or pharmacy for a SPACER to reduce or eliminate aerosol particles in the air during inhaler use.
We cannot accept nebulizer orders for school this year.
- Epi Pens, inhalers, diabetes care and seizure medications require a specific district form(s) that must be completed by parent, doctor, and the School Nurse.
-Special medical orders are also needed for students who will require medical procedures while in school. Please contact the Health Office for more information.

3) P.E. exemptions or modifications:
- Any recurring P.E. or recess exemption exceeding 2 days requires a district form to be completed by your doctor. This also applies to physical activity modifications and protective gear. Please contact the Health Office to request this form.

4) Crutches/Wheelchair:

- Should your student need to attend school with crutches or a wheelchair, a doctor’s note is needed before the student returns to school.
5) ADDITIONAL INFORMATION:

- A Google survey will be emailed and/or shared with school parents in lieu of the blue Student Health Information cards this school year. Please be sure to complete the survey in order for the Health Office to update your students’ health record. You can also access the survey through the link on the left side of this page: STUDENT HEALTH INFORMATION LINK.

Update your student's Health information

Health Services has created an electronic version of the Student Health Information form.  Due to COVID-19, the health office will not be sending out paper copies. 

Please complete this survey if your student is new to the Clark County School District or there have been any health related changes. If the reported health condition(s) of your student has not changed there is no need to complete the survey at this time.
